We are able to file out-of-network claims for you for United, UMR, Optum, BCBS, Cigna, and Tricare. The filing of out of network claims is a courtesy extended to you by us, and in no way is an indication that your insurance will reimburse you for our fees. It is important to understand your insurance benefits, and to understand the difference between in network and out of network. We have provided a brief explanation below.
Insurance is an agreement between you and your insurance company as to how your counseling will be paid for. This benefits agreement can usually be found either on the website of your insurance carrier or by calling the number on the back of your insurance card. Insurance may cover some or all of the cost of therapy. Please call the number on the back of your insurance card to verify your behavioral health benefits. It is helpful to ask the following questions:
Do I have mental health insurance benefits?
What is my deductible and has it been met?
How many sessions per year does my health insurance cover?
What is the coverage amount per therapy session?
Is approval required from my primary care physician?
In Network
In-network means that your therapist has contracted with your insurance company to provide services for a contracted rate. If your therapist is in-network with your insurance, they will file claims for your insurance company, and you are responsible for copays and deductibles as they are determined by your insurance company. We are in-network with Humana Insurance.Out of Network
Out of Network means that your therapist does not have a contract with your insurance company. If your therapist is out of network then you will be responsible for payment for your session at the time of service. We are able to file claims for you out of network for several insurance companies. In this case, you are responsible for the full cost of the session, your therapist will file the claim for you, and any benefits paid by the insurance will be paid directly to you. In the event your therapist cannot file claims with your insurance, your therapist can give you a form called a “superbill”. A superbill is simply a specialized invoice that you can file with your insurance carrier to be reimbursed for some of the cost of your counseling session. -
Please view our individual provider pages for specific information on fees. Our sessions rates per provider are as follows:
Counseling Intake (Individual or Couple 60 min): $185
Individual, Couples or Family Session (60 min): $185
No Show or Late Cancellation (less than 24 hours): Full session fee
These are our standard rates, but we do offer reduced fee sessions on a limited basis. If you need to use the sliding fee scale, please discuss the fee with your therapist.
The No Surprises Act was enacted in 2020 with the goal of protecting patients from unexpected bills for healthcare services, such as charges for out-of-network emergency care. Many of its provisions do not apply directly to us as the providers at Mending Hearts & Minds, or to you as our patients. However, beginning January 1, 2022, we are, as mental health providers, required to provide you with a good faith estimate of the cost of our services if you are self pay or we are out of network for your insurance company. It is easy for you to estimate the cost of your therapy with us by simply multiplying the (cost of our sessions X the number of sessions) you attend. However, we will provide at the first session an estimate of cost of services. We will also upload that form into your patient portal where it will be accessible to you throughout your counseling.

Effective January 1, 2025, our late cancellation/no show policy for all appointments is as follows:
Cancellations without 48 hours notice will be charged the full session fee.
If you opt in for appointment reminders, and you have put your email address on file, you will receive an email reminder. You will be automatically opted in for email and text reminders when you complete your intake paperwork, and if you would like to opt out, you can do that in your patient portal on TherapyNotes, or we can do that for you.
If you cancel when you receive the email reminder, you should be within the 48 hour window. We do offer 2 “freebies” per year for late cancellations upon your request. We understand that emergencies happen: kids get sick; cars break down; accidents happen. The two “freebie” late cancellations cannot be used for “no show” appointments. We do not charge for cancellations due to your illness.
No shows to appointments will be charged regardless due to the impossibility of filling that time.
A valid credit card must be kept on file.
